ADOPTION APPLICATION

Instructions: This application is quite detailed and may take several minutes to complete; Please understand that this is necessary in order to determine the most appropriate placement for our rescue kitties. Read this application carefully, fill in each blank and its entirety. In order to process your application promptly, you must answer ALL questions, otherwise your application will not be processed, nor approved. The information you provide in this application and during our interview will help us find a good kitty match for you. Please allow at least three days for a response. Thank you!

IN ORDER TO BE CONSIDERED AS AN ADOPTER, YOU MUST MEET THE CRITERIA BELOW:

Be 21 years of age or older.
Have identification showing your current address
Have the knowledge and consent of your landlord (if applicable)
Be willing to make a life long commitment to the care and humane treatment of the pet you are about to adopt from S.W.R.O.
Sign an Adoption Contract upon placement of an animal from S.W.R.O.

IMPORTANT NOTE: If you have other cats in your household, you must show us proof via your veterinarian hospital that the cat or cats in your household have been tested for both Feline Leukemia and FIV. If you do not have proof of testing, you must agree to have your cats/kittens tested and must provide us with proof of negative results.

The animals available for adoption from S.W.R.O. come from a variety of sources. All of our rescue animals are examined and treated by licensed veterinarians and their health is continually monitored while under our care. Our adoption fee includes all age-appropriate vaccinations, examinations, de-worming medications, flea & tick topical, Feline Leukemia/FIV testing and the AVID microchip. All cats will be spayed or neutered prior to adoption (unless under 2 months of age).
